Tadalafil is a PDE5 inhibitor. PDE5 is an enzyme that ends the relaxing signal in blood vessel walls, so blocking it lets vessels stay open longer. It is sold as Cialis and Adcirca.

Tadalafil is one of a small group of medicines that changed how erectile dysfunction is treated. It was approved in 2003, after sildenafil.

Its distinguishing feature is duration. It stays active far longer than the others in its class, which earned it the nickname the weekend pill.

How it works

During arousal, nerves release a signal that makes blood vessel walls relax. Relaxed vessels let more blood in, which produces an erection.

An enzyme called PDE5 ends that signal. Tadalafil blocks the enzyme, so the relaxing signal lasts longer and blood flow is easier to maintain.

It does not create arousal. Without the initial nerve signal there is nothing for it to extend, which is why it only works alongside stimulation.
